Your First Appointment with Nose Creek Sport Physical Therapy
We recommend that you arrive 20 minutes early for CHR, WCB and Private patients, and 30 minutes for MVC patients, to allow yourself time to complete the appropriate paperwork which will aid us in your assessment. Or, you can download the Intake Form, fill it out and bring it with you. This will help expedite your first visit. Please have your doctor download the PT Referral Script, complete it and send it to us.
You will meet your Nose Creek physiotherapist who will begin your comprehensive exam by completing:
- your history of recent and/or previous injuries,
- general health, and
- recreational pastimes.
Your Nose Creek Physical Therapist will perform a comprehensive objective physical assessment: They will test: your joint movements, muscle strength and flexibility and nerve conductivity and mobility.
Once your Physical Therapist has isolated your condition, they will begin treatment. Tour Our Facilities to see the equipment and various treatments we have available to return you to optimum health.
Next specific exercises are prescribed that are geared toward your recovery.
Follow up treatments are scheduled to carefully monitor your safe and gradual increase in function.
