Claiming On Your Insurance
We accept payment from 4 different sources:
- Alberta Health Services (public funding fractures, surgeries, and low income),
- Private (pay and get reimbursed through extended health benefits),
- WCB (work related) and
- MVC (motor vehicle collision)
Some insurance programs will cover physical therapy treatments. Check your policy details to see if you can claim all or a portion of your treatments. Also, if you were in a Motor Vehicle Accident, you can usually claim your Physical Therapy on your Insurance.
For Motor Vehicle Claims
We will need from you:
- Your Insurance Company name
- The Name of your Adjuster
- Claim/Policy #
- Fax and phone # of your Adjuster.
You will need to complete an AB-1 form for the insurance company within 10 days of car accident to ensure we are able to set up direct billing through your insurance company. AB-1 forms are available at either one of our offices or at your insurance company.
We will need you to come in for your physical therapy appointment at least 30 minutes early so we are able to complete all forms necessary.
